VISIT LETTERS, IMMUNIZATION RECORDS AND PATIENT DOCUMENTS

Physical Exam Letters, Visit Notes and Immunization Records will ONLY be printed and  handed out IN OFFICE ONLY at the end of the Patient's Well Visit. For additional copies of these and other patient documents, please refer to the Patient Portal. For questions or instructions on how to access our PATIENT PORTAL, please speak to the Front Desk or Practice Manager.

If you are interested in same-day COVID vaccine appointments for 5-11 year olds , please call the office between 2 and 3pm to see if we have any available same-day vaccines available. We also still have availability for our December 18th and January 8th Covid Vaccine clinic for 5-11 year olds.

Prepare for your child’s vaccination visit.

  • Get tips for how to support your child before, during, and after the visit.
  • Talk to your child before the visit about what to expect.
  • Tell the doctor or nurse about any allergies your child may have.
  • Comfort your child during the appointment.
  • To prevent fainting and injuries related to fainting, your child should be seated or lying down during vaccination and for 15 minutes after the vaccine is given.
  • After your child’s COVID-19 vaccination, you will be asked to stay for 15–30 minutes so your child can be observed in case they have a severe allergic reaction and need immediate treatment.
