How can I contact the office?

Our phone number is (508) 394-2116 and our phones are open from 8 am to 5 pm Monday through Friday and 8 am - 12 noon on weekends. After hours calls will be forwarded to our answering service who will page the on-call doctor and you should receive a call back from the doctor within an hour. Where is your office located?

Does my child need a new physical?

The Appointment / Vaccine Schedule gives the timeline for all of your child's visits from their 2 week visit to their 18 year physical. From ages 6-18 the American Academy of Pediatrics requires a physical every 2 years, but many patients will need a yearly physical to participate in sports or camps.

 

Am I going to get a shot!?

The Appointment / Vaccine Schedule shows the standard progression of vaccinations for a child who is up-to-date with all of their shots. We encourage all of our patients to vaccinate. Our vaccination program is based on the mandated Vaccination schedule by the Commonwealth of Massachussetts. How can I get my child’s school and camp forms filled out?

Forms can be mailed,  dropped off, or faxed to our office at (508) 760-1919. If your school or camp doesn't provide a form a copy of the patients immunization record and most recent physical can be requested by phone. Please allow 24-48 hours for all forms to be completed. Completed forms can be mailed or faxed to a provided address or they can picked up at our office or be made available through the Patient Portal.

How can I transfer myself or my child out of the practice?

To transfer a patient out of the practice we need written authorization from the patient or from a parent if the child is under the age of 18. The authorization must contain the patient's name, date of birth, the name and address where the records need to be sent, and a signature.  If you are only requesting a copy of your child's immunizations and last physical, this will be free of charge.  However, if you would like a copy of your child's complete medical record, the fee will be $15.00.  Please allow 5 business days for completion and note that we will not fax records. Please be awrae that we currently make it a practice to not switch Providers once you are connected with a Provider within our practice. Where can I get more information about my child's Lead Test or about lead exposure?
